Damages and issues 

Please inspect your order upon reception and contact us immediately if the item is defective, damaged or if you receive the wrong item, so that we can evaluate the issue and make it right. Please know that due to the nature of HDPE there will be light scuff marks on the material. If purchasing large sheets of HDPE for form kits or if you are custom building a form, please note that these bases may come not perfectly flat due to the nature of sheet goods.  These are not considered damage as they are part of the normal state of the material and will not negatively affect your pour. 

Returns & Cancellations
Returns are on a per item bases.  We do not accept returns on all items.  Returns must be requested in writing within 5 business days of delivery and if your product is eligible for return, a RMA number and return shipping address will be provided.  The customer is responsible for returning the product in the original packaging with the RMA number inside and a 10% restocking fee will be required.  You can requests information on returns by emailing at info@epoxyforms.com.

We will accept cancellations within 24 hours of the purchase, as long as the item has not already shipped. Please contact us at info@epoxyforms.com if you have specific questions or if you need to request a cancellation. Cancellations requested after 24 hours will not be processed.
